The Central African Republic has made significant progress in its response to HIV over the past four years. New HIV infections continue to decline and the number of people on antiretroviral therapy has increased from less than 25 000 in 2016 to more than 47 000 in 2019. Despite this progress, the Central African Republic remains far from the 90–90–90 targets. Less than half of the 100 000 people living with HIV in the country have access to treatment and the government estimates that less than 20% of people on treatment have an undetectable viral load. For those who are on treatment, the quality of care is insufficient. Recent studies reveal a very high rate of immunological and therapeutic failure among people living with HIV on treatment. Various factors contribute to this situation, including hospital-based management, the unavailability of biological monitoring in rural areas, the lack of human and material resources in health facilities and laboratories and the absence of community-based approaches that promote treatment adherence and information. In addition to these system-related factors, since December 2020 the country has faced a resurgence of a political and security crisis, which makes access to treatment difficult. There is a need to simplify and adapt the cascade of HIV-related services to take into account the preferences and expectations of different groups of people living with HIV while reducing unnecessary burdens on the health system. People-centred care is now the goal of the Central African Republic. The Central African Republic aims to diversify, accelerate and generalize differentiated services. The Catalytic Initiative promoted by UNAIDS with the financial support of Luxembourg is helping the country to achieve this ambition. A first decisive step has been taken with the organization of a round-table discussion, entitled Catalytic Initiative on Differentiated HIV and Tuberculosis Services. The meeting brought together the Ministry of Health, the Comité National de Lutte contre le SIDA, civil society, people living with HIV, tuberculosis stakeholders, key populations, youth organizations, the United Nations, Médecins Sans Frontières and the French Red Cross. The participants reviewed the existing pilot programmes of differentiated services in the country and the obstacles related to their implementation. They stressed the urgent need to expand differentiated services, particularly in the current context of the increasing insecurity and humanitarian challenges that make it difficult for people to access HIV and tuberculosis services. “It is in the current context of the security crisis that the differentiated services approach is most valuable in the Central African Republic,” said Patrick Eba, UNAIDS Country Director for the Central African Republic. The participants agreed on the activities to be implemented and validated the project implementation schedule to improve the quality of care and biological monitoring of people living with HIV and tuberculosis patients by stimulating the implementation of differentiated services. “I welcome this opportunity offered by UNAIDS to remove bottlenecks to improve the overall management of HIV,” said Marie-Charlotte Banthas Bata, the Director of Communicable Disease Control at the Ministry of Health and Population, Central African Republic, pictured above, centre. “Community organizations must be at the centre of the implementation of differentiated approaches,” said Welcome Gazalima, the President of the National Network of Associations of People Living with HIV in the Central African Republic The next key steps will be the documentation of existing differentiated approaches in the country and the development of a national guidance document for scaling them up.Distributed by APO Group on behalf of United Nations Programme on HIV/AIDS (UNAIDS).Media filesDownload logo

Download logoIn Kwa Bada village in north-eastern Tanzania, community health volunteer Sylvia Petro is hunting an ancient, endemic disease and shining a light on the devastating stigma that surrounds it. Leprosy has long plagued humanity yet has been treatable for decades by multidrug therapy. Described in the literature of ancient civilizations, sufferers are often shunned by fearful families and communities. Leprosy can cause permanent damage to the skin, nerves, limbs and eyes if left untreated. Yet when symptoms appear, patients often lock themselves away in shame, making it hard to treat and cure. Sylvia understands all this, as she has been treated for leprosy herself. “The promise of a cure helps some patients accept their situation but, in many cases, they still try and hide their worsening condition, even from their own families and in their own homes,” Sylvia says. “Family members of patients that seek treatment often cannot accept that a relative has leprosy.” Despite the challenge, Sylvia is trusted and well known in the over 500-household strong community. She goes from door to door, patiently encouraging people to seek treatment. “I want to make sure everyone in my district understands how Leprosy is spread. I want every single case in Muheza to be registered for treatment,” she says. Thanks to the World Health Organization (WHO)-supported multidrug therapy led by Tanzania’s National Tuberculosis and Leprosy Programme, the country brought the number of Leprosy cases down from nearly 35 000 in 1983 to under 1600 in 2019. The national prevalence has dropped to 0.3 cases per 10 000 people. The National Tuberculosis and Leprosy Programme also ensures timely case-finding and treatment, contact-tracing and household contact screening. Areas reporting high infections and disabilities are given priority. “We are at an encouraging stage in our effort to eliminate leprosy. We are working to integrate leprosy control in general health care services and apply cost-effective methods to improve community awareness and acceptance, and to combat stigma and discrimination against persons and families affected by leprosy,” says Dr Deus Vedastus Kamara, the National Leprosy and Tuberculosis Programme Coordinator. Fuelled by stigma and a lack of understanding in communities, the disease remains stubbornly endemic in Muheza and nine other districts. The last mile “I thought the spots would just go away,” says Belinda Sekioni, who is now on multidrug therapy. “I thought maybe a plant’s juices had reacted with my skin, or that I stayed in the sun too long on the farm.” “I’m thankful to the health workers who counselled me. The most important thing I learned was that leprosy is not spread down through families. This corrected my misunderstanding.” Sylvia Petro is one of 90 community health volunteers trained by National Tuberculosis and Leprosy Programme and WHO with support from the Nippon Foundation of the Sasakawa Health Foundation to end the disease in Tanzania in line with a declaration towards a leprosy-free world reached in Bangkok in 2013 by the Health Ministers from all high-burden countries. National Tuberculosis and Leprosy Programme is also strengthening monitoring and evaluation to collect better data to target responses, improve coordination and boost knowledge and awareness of the disease and available treatments. Since the project began in 2017, new cases in Muheza have halved to just 15 in 2020. Building on this success, Tanzania has now set up a 2021–2025 strategic plan to eliminate leprosy. Community health workers like Sylvia and WHO-supported training programmes to boost community awareness and help health workers correctly diagnose the disease are critical to the endeavour.    “Community awareness of the disease has increased in Muheza, bringing down the number of cases in Muheza district and reducing the number of patients that seek treatment after the disease has already advanced,” said Dr Jessie Singano, Muheza District’s Leprosy and Tuberculosis Response Coordinator.Distributed by APO Group on behalf of WHO Regional Office for Africa.

The inclusion and integration of women in sports has been a contentious topic over the years. As the industry continues to open space for women all around the world, International Women’s month is a good time to not only commemorate pioneers such as Abigail Kawonza, who have pushed boundaries and opened doors for other women – but to also celebrate the work done to date. Having been involved in rugby for over two decades – Abigail Kawonza has witnessed the industry’s many wins and losses. However, in her role as one of Rugby Africa (www.RugbyAfrique.com)’s Unstoppables, she has also seen and celebrated many developments, especially in a sport that was largely considered for men. Being part of the Unstoppables, a campaign dedicated to fast-tracking the development of women’s rugby in Africa, Kawonza pridefully notes that one of the major developments in her time is “the widespread acceptance of women taking part in rugby tournaments which has helped to project the sport onto higher platforms and expand its popularity. We have seen a change from just articles to women’s rugby being accepted in Olympics and Commonwealth Games.” Coralie van den Berg, General Manager of World Rugby African Association, Rugby Africa, testifies to this, explaining that increasingly “more tournaments including women’s categories are being developed. When unions now launch a new tournament, they tend to always include a women’s category which is a very positive development.” Kawonza notes that some of the major improvements she has also seen on a global scale is “the increase in funding covering the development of the game in both sevens and the fifteens format as well as younger and stronger players emerging.” As the industry increasingly makes provision for women, Kawonza also notes an increase in adequate skill levels to showcase great games at any level due to properly accredited coaches taking time to coach women. “Let’s not forget qualified female technical staff taking the reins” she adds. Kawonza’s passion for sports and her journey as an Unstoppable has also opened bigger opportunities for her, allowing her to make even wider strides towards opening the industry for other talented women rooting on the other side of the fence. One of the key achievements being her role as the President of the Zimbabwe Rugby Referees Association which has opened opportunities to better guide younger referees towards maintaining best practice. Beyond the amazing strides Kawonza has made in her career, her tenure as an Unstoppable has further expanded her impact beyond Zimbabwe’s grounds. “I appreciate that my influence has gone beyond the Zimbabwean borders as I also work closely with regional and international union boards”, says Kawonza. “The recognition as an Unstoppable has opened doors for more women in Zimbabwe, and I intend to ride the wave for as long as I can. Taking young match officials and players with me” she adds. In her Scrumqueens Q&#038;A, Maha Zaoui shares the same sentiment as Kawonza, adding that “the creation of the Women’s Rugby Advisory Committee at Rugby Afrique and its 4 sub-committees speaks to the global vision of women’s rugby in Africa. We are no longer talking about priorities but about a global project that will affect all related sectors in women’s rugby.” Kawonza’s key focus has been to bridge the gap between talent search and development and foster talent that can soar to professional rugby heights. About talent, Rugby Africa Referee Manager, Mudiwa Mundawarara shares that “over the years, the support of the South African Rugby Union and French Rugby Federation has been tremendous, and I am pleased that it will continue with greater emphasis on talent identification, on training and coaching of referees from the rest of the continent. This will eventually lead to greater opportunities for them through a selection process drawn from the strengthened development cohorts.” Beyond opening doors for young women, Kawonza’s role as an Unstoppable also enabled her to mentor upcoming players to be smart about their careers beyond the field. “I have a different view as an “older” if not the oldest Unstoppable &#8211; I encourage young women to pursue their education to the fullest because sport goes beyond what you do on the field, it’s about you being articulate and able to represent yourself as a brand in the world out there. This will help you go beyond the field when you reach your “shelf-life” or even if playing is not your thing but still have that passion for rugby” says Kawonza. To celebrate women’s day, the Uganda Rugby Union (www.UgandaRugby.com) organized a Level 1 coaching course for women at Kyadondo Rugby Grounds. The course was conducted by World Rugby Educators Helen Buteme and Andrew Owor. The level 1 course covered coaching tips, principles of play, roles of a coach, the different coaching styles and aims of coaching. A participant centered approach for the 16 female rugby players was used to ensure proper understanding of the course elements. The course involved many practical sessions to do with ball reception kick off, line outs, clearing a pass, a swab, mauling, drop kicks. Nairobi has 443 cases, Turkana 45, Kiambu 42, Nakuru 36, Meru 24, Kajiado 13, Machakos 13, Kisii 12, Uasin Gishu 10, Kisumu 9, Garissa 9, Siaya 8, Kilifi 7, Makueni 7, Busia 7, Kericho 5, Nyandarua 4, Kakamega 3, Taita Taveta 3, Kitui 2, Mombasa 2, Murang’a 2, Vihiga 2, Nyeri 1, Bungoma 1, Embu 1, Kirinyaga 1 and Kwale 1. 167 patients have recovered from the disease, 115 from the Home Based and Isolation Care while 52 are from various health facilities. Total recoveries now stand at 87,903. 12 patients have succumbed to the disease pushing our cumulative fatalities to 1,898.  Our sincere condolences to the families and friends of those who have lost their loved ones. 563 patients are currently admitted in various health facilities countrywide. 1,588 patients are on Home Based Isolation and Care.   89 patients are in the ICU, 21 are on ventilatory support. 59 on supplemental oxygen.  9 patients are on observation. 28 patients are separately on supplementary oxygen with 26 of them in the general wards.  Two patients are in the High Dependency Unit.
